For the Personal Care Home:
Understanding the specific care needs of prospective residents is crucial for Personal Care Homes to make informed admission decisions. By gaining detailed knowledge through our assessment process, Personal Care Homes can determine if they are equipped to provide the appropriate level of care that meets the individual’s needs.
In compliance with Saskatchewan Personal Care Homes Regulations (Part III 14(1)), we will conduct the initial admission assessment which is required to be arranged within 7 days of a resident entering the home.
Our assessment program is based on a globally recognized healthcare assessment tool. This tool comprehensively gathers information on the resident’s medical health status, functional abilities, cognitive function, and psychosocial well-being.
A thorough assessment serves as a cornerstone for developing precise, individualized care plans. This process is integral to delivering person-centered care within Personal Care Homes, ensuring that each resident receives tailored support based on their unique needs.
By conducting assessments at regular intervals, Personal Care Home staff can monitor changes in the resident’s health status and track the effectiveness of care plan interventions over time to ensure the resident’s needs are being met.
Saskatchewan Personal Care Homes Regulations, Part III 14(2) A licensee shall request a written reassessment of a resident by an assessment agency.
(a) whenever the care needs of the resident change.
(b) no later than 2 years after the date of the resident’s last assessment.

Alzheimers Disease, Vascular Dementia, Normal Pressure Hydrocephalus, Mixed Dementia. Strategies to help manage wandering, resistance to care, sleep disturbances, sundowner’s syndrome and repetitive disruption. Certificate of completion.
Huntington’s Disease, Lewy Body Dementia, Parkinson’s Disease and Frontal Temporal Lobe Dementia. Strategies to help manage agitation/aggression, hallucinations/delusions/paranoia, catastrophic reactions and sexually inappropriate behaviour. Certificate of completion.
For Residents who require more than just basic personal care, Personal Care Homes are required to arrange training by a health care professional for their caregivers to provide that care specifically for the individual resident.
Proper training ensures that staff are knowledgeable about these conditions and how to provide appropriate care and support.
Senior Care Compass will provide an assessment of care required, in-depth teaching and required documentation by a Registered Nurse or Licensed Practical Nurse.
