For the potential resident:
Senior Care Compass offers a professional pre-admission assessment service tailored for potential residents. Our team comprises experienced assessors who understand the challenges faced by seniors, caregivers, and families alike. Through a thorough assessment, we identify each individual’s current strengths, abilities, and care needs. Combined with our Selecting a Personal Care Home Guide, this assessment equips you with the information needed to find a Personal Care Home that aligns with your preferences, financial considerations, and care requirements.
For the Personal Care Home:
Understanding the specific care needs of prospective residents is crucial for Personal Care Homes to make informed admission decisions. By gaining detailed knowledge through our assessment process, Personal Care Homes can determine if they are equipped to provide the appropriate level of care that meets the individual’s needs.
In compliance with Saskatchewan Personal Care Homes Regulations (Part III 14(1)), we will conduct the initial admission assessment which is required to be arranged within 7 days of a resident entering the home.
Our assessment program is based on a globally recognized healthcare assessment tool. This tool comprehensively gathers information on the resident’s medical health status, functional abilities, cognitive function, and psychosocial well-being.
A thorough assessment serves as a cornerstone for developing precise, individualized care plans. This process is integral to delivering person-centered care within Personal Care Homes, ensuring that each resident receives tailored support based on their unique needs.
By conducting assessments at regular intervals, Personal Care Home staff can monitor changes in the resident’s health status and track the effectiveness of care plan interventions over time to ensure the resident’s needs are being met.
Saskatchewan Personal Care Homes Regulations, Part III 14(2) A licensee shall request a written reassessment of a resident by an assessment agency.
(a) whenever the care needs of the resident change.
(b) no later than 2 years after the date of the resident’s last assessment.
At Senior Care Compass, we recognize that each resident possesses unique needs, preferences, and care objectives. Our personalized care plans are meticulously crafted to deliver tailored interventions that address specific challenges while promoting health, dignity, and overall well-being.
Our Individual Resident Care Plans are informed by comprehensive assessments, encompassing activities of daily living, personal care requirements, social and spiritual needs, cognitive function, medical treatments, conditions, nutritional considerations, communication preferences, and both Dementia and non-Dementia-related concerns. We also incorporate assessments for transfers, mobility, and manage conditions such as pain, endurance, skin integrity, edema, and fall risks.
By comprehensively addressing the physical, emotional, and social dimensions of care, our care plans provide caregivers with a clear and consistent framework. This ensures that interventions are aligned with each resident’s needs, enhancing their quality of life and fostering maximum independence.
Care plans serve as essential tools for caregivers, guiding them in delivering personalized care that respects each resident’s goals and preferences. At Senior Care Compass, we are committed to ensuring that every resident receives the highest standard of care tailored to their individual needs and aspirations.
